Alcohol and Cancer Risk: What the Science Says
Alcohol causes cancer, and the risk starts with the first drink rather than only at heavy levels. The International Agency for Research on Cancer classes alcoholic drinks as a Group 1 carcinogen, the same category as tobacco and asbestos, with enough evidence to link it to at least seven cancers. There is no safe amount for cancer, so the lowest-risk choice is none, and the good news is that risk drops the longer you stay stopped.
Here is the whole picture in one paragraph. In 2020, alcohol was behind an estimated 741,300 new cancer cases worldwide, about 4.1 percent of all new cancers, according to a study in The Lancet Oncology led by IARC researchers. The seven cancers with sufficient evidence of a causal link are cancers of the mouth, throat (pharynx), voice box (larynx), oesophagus, liver, bowel (colorectum), and female breast. Heavy drinking of more than two drinks a day drove 86 percent of those cases, but light to moderate drinking still accounted for roughly one in seven, over 100,000 cases. Alcohol turns into acetaldehyde in the body, a chemical that damages DNA, which is the main way it seeds cancer.
What the science actually says
The core finding is blunt: for cancer, no level of alcohol is safe. Both the WHO and IARC now frame the guidance as "less is better, none is best" rather than pointing to a threshold below which drinking is harmless. That marks a shift from older "moderate drinking is fine" messaging, and it rests on decades of studies showing risk rising in step with intake for several cancers, with no floor where the extra risk disappears.
This doesn't mean one glass of wine gives you cancer. It means alcohol nudges the odds up, a little at low intake and a lot at high intake, across a whole population. The mouth, throat, breast and bowel figures come from a large dose-response meta-analysis in the British Journal of Cancer; the liver figure comes from a separate liver-cancer meta-analysis, where risk keeps climbing with intake rather than settling at one multiplier. A number like "5 times" sounds alarming, so keep it in proportion: it multiplies a baseline risk that is itself modest for most of these cancers. The point is the direction and the steepness, not a personal verdict.
Breast cancer deserves a closer look because it is where alcohol quietly does the most damage across a population. Its relative risk is small per drink, but breast cancer is common, so even a modest bump translates into a large number of cases. IARC's own breakdown found breast cancer was the leading alcohol-attributable cancer in women worldwide. That is the pattern to hold onto: a steep link in a rare cancer and a shallow link in a common one can end up causing similar numbers of cases.
How does alcohol trigger cancer
Alcohol doesn't cause cancer through one clean pathway. It works through several at once, which is part of why the evidence is so consistent. The main mechanism is acetaldehyde. When your body breaks down ethanol, it first makes acetaldehyde, which the IARC also classes as carcinogenic. It binds to DNA and blocks the repair machinery, letting damaged cells slip through.
There is more. Alcohol generates reactive oxygen species that add oxidative stress to tissues. It raises levels of oestrogen and other hormones, which helps explain the breast cancer link. In the mouth and throat it acts as a solvent, letting carcinogens from tobacco smoke soak into cells more easily, so drinking and smoking together multiply the risk rather than just adding it. People who both smoke and drink heavily face a far higher risk of mouth and throat cancer than you would get by summing the two habits separately. Alcohol also impairs the absorption of folate and other nutrients the body uses to keep DNA intact, which leaves cells with fewer of the tools they need to fix everyday damage.
Is there a safe amount to drink
For cancer specifically, no. Breast cancer is the clearest example of risk with no safe floor: studies find the odds tick up even at less than one drink a day, and each additional daily drink adds a few more percent. That is why the light-to-moderate band, drinking most people think of as harmless, still produced more than 100,000 alcohol-linked cancers in a single year.
"No safe level" is not the same as "any drink is dangerous," and it helps to be precise about that. Health bodies now use "low-risk, not safe" language for their weekly guidelines, meaning the limits are set to keep risk small, not to promise none. How much does the risk rise with heavier drinking

Compared with a non-drinker, the relative risk of liver cancer is about 1.08 at one drink a day, roughly 1.5 at three to four drinks, about 2.1 at five, and over 3 at seven a day, based on a published liver-cancer meta-analysis. The curve keeps steepening from there.
Heavy drinking is where most of the harm sits. The Lancet Oncology analysis attributed 86 percent of alcohol-caused cancers to risky and heavy drinking of more than two drinks a day. This is the same intake band that drives liver damage and raised blood pressure, so cutting back from heavy to light pays off across several fronts at once, not just cancer.
Because the curve steepens, cutting back matters most for the heaviest drinkers, where each drink removed takes off a larger slice of risk than it would for someone already drinking little. That is worth knowing if the "no safe level" message feels like all-or-nothing. It is not. Moving from six drinks a day to two is a real reduction, even if it is not zero, and it lands in the part of the curve where the payoff is biggest.
Does cancer risk fall after you stop drinking
Yes, though slowly, and the benefit grows with every year alcohol-free. A 2024 IARC review in the New England Journal of Medicine concluded there is now sufficient evidence that stopping or reducing alcohol lowers the risk of mouth and oesophageal cancers. For mouth cancer, studies find risk about 19 percent lower after up to four years without drinking, about 23 percent lower at five to nine years, and roughly 55 percent lower after twenty years, drifting back toward the risk of someone who never drank.
Liver cancer follows the same shape more gradually, easing by an estimated 6 to 7 percent a year after you stop, so it takes many years to approach never-drinker levels. The lesson is not "the damage is done." It is that time off alcohol keeps paying out, and the sooner you start, the sooner the clock starts.
