Liver Pain After Quitting Alcohol: Causes and What to Do

Liver pain after quitting alcohol is usually a dull ache or fullness under your right ribs. It can come from a swollen liver, but the gallbladder, pancreas, stomach and even a strained muscle can hurt in the same spot. It isn't a reliable sign of healing, so pain that lasts more than a few days deserves a GP visit and a liver blood test, and yellow skin, fever, vomiting blood or confusion need emergency care.

Fat builds up in the liver of 90 to 95% of people who drink heavily over a long period, and 20 to 40% of them go on to develop scarring (fibrosis), according to a 2022 review in Alcohol Research: Current Reviews. The NHS says most people with alcohol-related liver disease feel nothing until the damage is advanced, with vague early symptoms: belly pain, poor appetite, tiredness and nausea.

Early fatty liver can return to normal after about two weeks without alcohol, according to the NHS. Alcohol-related hepatitis behaves differently. It is defined by jaundice appearing within 60 days of heavy drinking, so it can surface after someone has already stopped (StatPearls).

Where is your liver, and what does liver pain feel like?

Your liver sits in the upper right part of your abdomen, tucked mostly behind your lower right ribs.

The NIDDK describes liver discomfort as mild pain over the upper right side of the abdomen. People usually call it:

  • a dull, heavy ache rather than a stabbing pain
  • a feeling of fullness or pressure under the right ribs
  • a soreness that's worse when you press there or wear a tight waistband

The ache can spread toward the right shoulder or back, but so can pain from nearby organs, so location alone can't tell you what's wrong.

Why does my liver hurt after I stopped drinking?

Several explanations fit, and more than one can apply.

A swollen liver stretching its covering. Liver tissue itself has very few pain-sensing nerves. Its thin outer covering, called Glisson's capsule, does. When a liver is enlarged by fat or inflammation, the capsule stretches and you feel an ache. That swelling can take weeks to settle, so the ache may outlast your last drink.

You're finally noticing it. Alcohol dulls pain. Sober, and often unsettled by withdrawal in the first days, many people notice twinges that were there all along.

Alcohol-related hepatitis showing up late. This is the one not to miss. StatPearls notes that the NIAAA definition of alcohol-related hepatitis is jaundice starting within 60 days of heavy drinking (more than 50 g of alcohol a day for at least six months). A US standard drink holds 14 g of alcohol and a UK unit 8 g, so fifty grams is about 3.5 US drinks or just over 6 UK units.

So someone who drank heavily can develop it a few weeks into sobriety, with right-sided pain, fever and yellowing skin. That needs a doctor quickly.

A neighbouring organ. Gallstones, an irritated stomach lining and pancreatitis all cause upper belly pain.

Does liver pain mean your liver is healing?

There's no good evidence for this popular forum claim. Liver healing is mostly silent, while pain over the liver appears in conditions that need attention, such as alcohol-related hepatitis and cirrhosis (NIDDK, StatPearls).

Pain says something is worth a look. A blood test and sometimes a scan show what it is. For the healing side of the story, how the liver recovers after quitting alcohol walks through each stage and how long it takes.

Per the NHS, gallstone pain that lasts longer than five hours, or comes with a high temperature, may mean an inflamed gallbladder and needs urgent care. Alcohol-related pancreatitis pain often starts 6 to 12 hours after heavy drinking, so it can begin the day after a final binge.

A dull ache that fades steadily over the first week or two, with no red flags, is the least worrying pattern. Pain that's still there after that, or getting worse, means booking a GP appointment rather than waiting.

When is pain under the right ribs an emergency?

Get emergency help (call your local emergency number or go to A&E or the ER) if pain comes with any of these:

  • Yellow skin or yellow whites of the eyes (jaundice) or very dark urine, especially if it's new or getting worse quickly
  • Vomiting blood, or black, tarry stools
  • Confusion, unusual drowsiness or personality change
  • A high temperature or shivering with the pain
  • Severe pain that is constant, getting worse, or lasting more than five hours

A swollen belly or new swelling in the legs and ankles isn't usually an ambulance call, but it needs a GP (or NHS 111) the same day.

The NIDDK lists confusion, vomiting blood, rectal bleeding and rapidly worsening jaundice as reasons to seek emergency care in liver disease, and darker urine among the signs of advancing cirrhosis. The NHS adds very itchy skin, swelling from fluid build-up in the legs or belly, and fever with shivering to the signs of more advanced alcohol-related liver disease.

Withdrawal is a separate emergency risk. If you drink heavily every day, stopping suddenly can trigger dangerous alcohol withdrawal, including seizures and delirium tremens. Don't quit cold turkey without medical guidance.

What will a doctor check for liver pain?

Liver function tests. A blood panel that usually includes ALT, AST, GGT and bilirubin. Raised enzymes point to liver stress, and bilirubin relates to jaundice. A test now also gives you a baseline for later.

An ultrasound scan. This shows fat in the liver, the liver's size, and gallstones, all in one look.

A FibroScan. A quick, painless scan that estimates liver stiffness to check for scarring, which the NHS lists among the tests for alcohol-related liver disease.

Honest answers help most. Tell your doctor roughly how much you drank and for how long, the date of your last drink, when the pain started, and what makes it better or worse.
